Reason for Change of Status as an Employer-sponsored Insurance Subscriber
- The status of an employer-sponsored insurance subscriber shall change on any of the days specified below (Subparagraphs 1 to 4 of Paragraph 1 of Article 9 and Subparagraph 2 of Article 7 of the National Health Insurance Act).
· The day on which a regional insurance subscriber becomes a national health insurance subscriber under a qualifying business or becomes a worker, public worker, or teacher (hereinafter referred to as a "worker, etc.")
· The day on which an employer-sponsored insurance subscriber becomes a national health insurance subscriber under another qualifying business or becomes a worker, etc.
· The day after the day on which the employment of a worker, etc. who is an employer-sponsored insurance subscriber ends
· The day after the day on which a qualifying business suspends or ends its operation
Declaration of Change of Status as an Employer-sponsored Insurance Subscriber
- If the status of an employer-sponsored insurance subscriber changes, a user who is an employer-sponsored insurance subscriber and a household head who is a regional insurance subscriber must declare the change to the Service within 14 days after the change occurs, as specified below (Paragraph 2 of Article 9 of the National Health Insurance Act and Paragraphs 1 and 2 of Article 4 of the Enforcement Regulations of the National Health Insurance Act):

▪ Declaration of Acquisition of or Change in the Status of a Regional Insurance Subscriber (Attached Template No. 5 of the Enforcement Regulations of the National Health Insurance Act) ▪ Proof of insurance premium exemption ※ If a user submits a Declaration of Loss of the Status of an employer-sponsored Insurance Subscriber (Attached Template No. 8 of the Enforcement Regulations of the National Health Insurance Act) to the Service, it is assumed that he or she submitted a Declaration of Acquisition of or Change in the Status of a Regional Insurance Subscriber.

Time of Loss of the Status of an Employer-sponsored Insurance Subscriber
- The status of an employer-sponsored insurance subscriber shall be lost on any of the days described below (Subparagraphs 1, 2, 5 and 6 of Paragraph 1 of Article 10 of the National Health Insurance Act):
· The day after the day on which an employer-sponsored insurance subscriber dies
· The day after the day on which an employer-sponsored insurance subscriber loses his or her Korean nationality
· The day on which an employer-sponsored insurance subscriber becomes a medical aid recipient in accordance with the Medical Care Assistance Act
· The day on which a health insurance benefits receiver applies to opt out of health insurance after becoming eligible for medical aid in accordance with the Act on the Honorable Treatment of Persons of Distinguished Services to Independence or the Act on the Honorable Treatment and Support of Persons, Etc. of Distinguished Service to the State
Declaration of Loss of the Status of an employer-sponsored Insurance Subscriber
- When a user loses the status of an employer-sponsored insurance subscriber, he or she must submit a Declaration of Loss of the Status of an employer-sponsored Insurance Subscriber to the Service within 14 days after such loss of status occurs (Paragraph 2 of Article 10 of the National Health Insurance Act and Subparagraph 2 of Paragraph 4 of Article 4 and Attached Template No. 8 of the Enforcement Regulations of the National Health Insurance Act).
